Kannon Health is required by law to keep your Protected Health Information (PHI) private, provide you with this Notice, and follow its terms. We reserve the right to update this Notice and will make updated versions available upon request or at our clinic locations.

How We May Use or Share Your Information

We may use or disclose your PHI for the following purposes:

1. Treatment

To provide, coordinate, or manage your healthcare and related services.

  • Example: Sharing vaccine records with your doctor.
     

2. Payment

To bill and receive payment from Medicare, Medicaid, insurance companies, or other payers.

  • Example: Submitting a claim for your vaccine.
     

3. Healthcare Operations

To run our organization and ensure quality care.

  • Example: Training staff or performing quality assessments.
     

4. State Immunization Registry Reporting

We may report your vaccines to the Texas Immunization Registry (ImmTrac) as allowed by law.

5. When Required by Law

We may disclose your PHI when required by federal, state, or local law.

6. Public Health Reporting

To prevent or control disease, report adverse events, or support public health investigations.

7. Health Oversight Activities

For audits, inspections, certifications, or investigations required by agencies such as the Department of Health and Human Services and the Center for Medicare & Medicaid Services (CMS).

8. Legal Matters

We may share PHI to comply with court orders or laws.

​

9. Workers’ Compensation and Employer Clinics

When vaccines are provided through an employer or facility, we may confirm that you received a vaccine. No other medical details will be shared.

Uses and Disclosures That Require Your Authorization

We will not share your information without your written consent for:

  • Marketing communications not related to your care

  • Sale of your PHI

  • Most sharing of psychotherapy notes
     

You may revoke your authorization at any time in writing.

Your Rights Regarding Your PHI

You have the right to:

1. Access Your Records

Request to see or receive a copy of your PHI.

​

2. Request a Correction

If something is inaccurate or incomplete, you can request an amendment.

​

3. Request a Restriction

Ask us to limit how your PHI is used or shared. We are not required to agree.

​

4. Request Confidential Communications

Request that we contact you in a specific way (example: by mail only).

​

5. Get a List of Disclosures

Request a list of times we have shared your PHI in the past six years.

​

6. Receive a Paper or Electronic Copy of This Notice

You may request a copy at any time.

​

 

How We Protect Your Information

We follow industry-standard safeguards to keep your information secure, including:

  • Employee training

  • Secure physical and electronic systems

  • Access limited to those with a need to know
