Kannon Health is required by law to keep your Protected Health Information (PHI) private, provide you with this Notice, and follow its terms. We reserve the right to update this Notice and will make updated versions available upon request or at our clinic locations.

4. State Immunization Registry Reporting
We may report your vaccines to the Texas Immunization Registry (ImmTrac) as allowed by law.

9. Workers’ Compensation and Employer Clinics
When vaccines are provided through an employer or facility, we may confirm that you received a vaccine. No other medical details will be shared.

Most sharing of psychotherapy notes
You may revoke your authorization at any time in writing.

If something is inaccurate or incomplete, you can request an amendment.

3. Request a Restriction
Ask us to limit how your PHI is used or shared. We are not required to agree.

Request that we contact you in a specific way (example: by mail only).

5. Get a List of Disclosures
Request a list of times we have shared your PHI in the past six years.

6. Receive a Paper or Electronic Copy of This Notice
You may request a copy at any time.
